Our palliative care service aims to support you during the difficult time, when you or your loved ones are suffering from a life-threatening illness. Care Matters' care service supports vulnerable people who have been diagnosed with a life threatening illness, and who wish to remain living in their own home.
Our aim is to achieve the best quality of life for you or your loved ones, and we understand that family and friends will be affected by the illness and it will be a difficult time for all and we will be sensitive to allow discussion and provide the support needed. We will provide a care plan that will be person centred to the needs of the Service User which will include their personal choices and preferences, and to ensure we provide continuity of care at all times by providing a small amount of Carers to get to know the Service Users personality and preferences on an individual basis.
Our service will be ongoing through all stages of care, whether that be caring for someone with a long-term condition or illness can last for a short period of time or many years. In the early stages our carers can also assist when attending hospital appointments, or just being there to support when good or bad news is received. We can also provide care and support following treatments to ensure we promote as much independence as possible, this includes personal care and social companionship. This can be provided to suit the Service Users convenience, from shorter calls through to 24 hour care.
Our personal care service can support you with morning and evening calls, bathing, dressing, continence care, medication prompts and meal preparation.
We can provide a Support Worker to sit, chat and read, providing the company needed to help the Service User feel comfortable and at ease.
24 Hour Care
Care Matters can provide 24 hour care as required, this can be useful to relatives or other support to enable them to get a good nights sleep as the Service Users condition deteriorates.
We are able to work with other agencies such as Medical professionals, Social Services, Local Authorities or charitable bodies, to ensure a consistent approach to care.
We offer an emergency response service to support Service Users during times of crisis. We understand that we may be needed to react quickly and may need a care package to start immediately. In such circumstances we will carry out an emergency assessment in order to facilitate a faster response. We will work in partnership with other healthcare professionals such as GPs and hospital Doctors, to ensure that all care needs are met.
